I inserted chSysIntegrityCheckI(CH_INTEGRITY_RLIST) in main thread.
After the thread was frozen I checked the result of this function and it is false.
In this moment there are one thread in Ready List and it is the idle thread because its priority is one.
It seems my thread is not in Ready List but state variable says another.
If I am true the thread was blocked after function chThdSleepMicroseconds(500);
What can prevent the thread from being placed on the Ready List after waking up?

It is very strange because the function chSchReadyI() sets the state AND places the function in the list so it should not happen.
Are all your IRQs in a valid priority range? priorities 0..2 should not be used for calling RTOS functions because the kernel can be preempted.
What I-class/S-class functions are you using, I would look around those. Do you do things from the idle thread?

I experimented many hours and days, even I can reproduce the problem with 100% repeatition but coud not understand what is happen. After that I found that after I changed default ST priority from 8 to 3 the problem was gone. This is the most strange thing with I met when using the ChibiOS in all time.
My project too complex and I tried to reproduce the problem with more simple program but could 't. Any change to the program is changing the reaction and time of the problem or it is disappeared.

Try setting CORTEX_SIMPLIFIED_PRIORITY to TRUE and see if it makes a difference.
