EXT is no more supported on STM32, the new PAL driver includes most functionalities.

Not yet, you could give a try to the OTGv1 driver but I haven't tested it already.

I committed a demo for STM32H7 using the OTGv1 driver, it compiles (after some changes) but it still does not work.
OTG registers always read back as zero like it is, somehow, not reachable, I suspect there is some missing configuration or configuration error. OTG clocks are enabled, I am stuck...
Has anybody managed to enable OTG already?

CAN and MAC are whole new drivers, I have no immediate plans for this. Long term, probably yes.

Is it the \testhal\STM32\multi\USB_CDC one?
I've compiled it for H7 nucleo from trunk, but the port not even appears in the system. There are no pull-up on D+ when USB is connected.
Cubemx generated project with USBCDC works on the same board. I've only have to set USB33DEN manually on usb start.
